What's the difference between total testosterone and free testosterone?

All of the testosterone in your blood—both the active and the bound to proteins like SHBG and albumin—is called total testosterone. Men can have low free testosterone despite normal total testosterone if SHBG is high. This test includes both figures, as well as SHBG and albumin, to give a true indication of how much usable testosterone your body actually has.
